A. Program Director/designee will be responsible for:

orientating all department heads of the plan
receiving communication regarding disaster and disseminating it
activating and discontinuing plan
developing an information/notification plan
coordinating the activities of key personnel

B. Director Patient Services/Assistant Director Field Operations will be responsible for:

ensuring all employees are oriented to plan and potential activities for their position
coordinating activities of para/professional staff
maintaining accurate listing of active clients and employees (address and phone numbers) at all times
maintaining accurate listing of clients coded RED and YELLOW

C. The Personnel Manager/designee will be responsible for informing all new employees regarding the agency's disaster response process and their responsibilities to the client and the agency:

call agency immediately after hearing sirens, weather warnings, civil emergency
stay with client until alternate care is in place (another aide, relative, caregiver, transfer to health facility)
stay away from disaster area and notify supervisor immediately
when unable to visit assigned client due to extreme emergency, notify agency immediately
participate in telephone triage

D. The Field Supervisors/designee will be responsible for:

educating client/client family regarding disaster/emergency preparedness
explaining responsibilities of client/client's family during a disaster/emergency
determining and explaining emergency coding/evacuation coding to client/client family
informing client/client family regarding agency's response to disaster/emergencies
